Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that supports research on pragmatic trials. It gathers and distributes clean trial data, ratings, and evaluations using PRECIS-2. This allows for diverse meta-epidemiological analyses that evaluate the effects of treatment across trials with different levels of pragmatism.
Background
Pragmatic trials provide evidence from the real world that can be used to make clinical decisions. However, the use of the term "pragmatic" is not consistent and its definition and evaluation requires further clarification. Pragmatic trials should be designed to inform clinical practice and policy decisions, rather than to prove the validity of a clinical or physiological hypothesis. Furthermore, pragmatic trials should seek to make their results as relevant to actual clinical practice as is possible by making sure that their primary method of analysis is based on the intention-to-treat method (as described in CONSORT extensions for pragmatic trials).
Despite these requirements, a number of RCTs with features that defy the notion of pragmatism were incorrectly labeled pragmatic and published in journals of all kinds. This could lead to misleading claims of pragmatism, and the use of the term should be standardized. The creation of a PRECIS-2 tool that provides an objective, standardized evaluation of pragmatic aspects is the first step.
Methods
In a pragmatic research study the aim is to inform clinical or policy decisions by showing how an intervention could be integrated into routine treatment in real-world settings. This differs from explanation trials that test hypotheses regarding the causal-effect relationship in idealized settings. In this way, pragmatic trials may have lower internal validity than explanation studies and be more prone to biases in their design as well as analysis and conduct. Despite their limitations, pragmatic research can be a valuable source of information to make decisions in the context of healthcare.
The PRECIS-2 tool scores an RCT on 9 domains, with scores ranging between 1 and 5 (very pragmatist). In this study, the recruit-ment organization, flexibility in delivery, flexible adherence and follow-up domains scored high scores, however the primary outcome and the method for missing data were below the limit of practicality. This suggests that it is possible to design a trial using high-quality pragmatic features, without harming the quality of the outcomes.
However, it is difficult to judge how pragmatic a particular trial is, since the pragmatism score is not a binary quality; certain aspects of a trial can be more pragmatic than others. Furthermore, logistical or protocol modifications made during the trial may alter its score on pragmatism. Results

It is important to understand that the term "pragmatic trial" does not necessarily mean a low quality trial, and there is an increasing rate of clinical trials (as defined by MEDLINE search, however this is neither specific or sensitive) which use the word "pragmatic" in their abstracts or titles. These terms may signal a greater appreciation of pragmatism in abstracts and titles, but it's not clear if this is reflected in the content.
Conclusions
As the importance of real-world evidence grows widespread, pragmatic trials have gained popularity in research. They are randomized studies that compare real-world alternatives to new treatments that are being developed. They involve patient populations more closely resembling those treated in regular medical care. This method can help overcome the limitations of observational research, for example, the biases that are associated with the reliance on volunteers, and the limited availability and the coding differences in national registry.
Other advantages of pragmatic trials are the ability to use existing data sources, and a higher chance of detecting meaningful changes than traditional trials. However, they may still have limitations that undermine their reliability and generalizability. Participation rates in some trials could be lower than anticipated due to the health-promoting effect, financial incentives or competition from other research studies. The necessity to recruit people quickly restricts the sample size and the impact of many pragmatic trials. Some pragmatic trials also lack controls to ensure that observed differences aren't due to biases in the trial.
The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-described themselves as pragmatist and published up to 2022. The PRECIS-2 tool was employed to assess the pragmatism of these trials. It covers areas such as eligibility criteria, recruitment flexibility and adherence to intervention and follow-up. They discovered that 14 of these trials scored as highly or pragmatic practical (i.e. scoring 5 or higher) in one or more of these domains and that the majority of them were single-center.
Studies with high pragmatism scores are likely to have more criteria for eligibility than conventional RCTs. They also contain populations from many different hospitals. The authors suggest that these characteristics can help make pragmatic trials more meaningful and applicable to everyday practice, but they do not necessarily guarantee that a pragmatic trial is completely free of bias. The pragmatism principle is not a fixed characteristic the test that doesn't have all the characteristics of an explanatory study could still yield reliable and beneficial results.
