Pragmatic Free Trial Meta
Pragmatic Free Trial Meta is a non-commercial, open data platform and infrastructure that supports research on pragmatic trials. It shares clean trial data and ratings using PRECIS-2 allowing for multiple and diverse meta-epidemiological studies that evaluate the effect of treatment on trials that employ different levels of pragmatism and other design features.
Background
Pragmatic trials are increasingly recognized as providing real-world evidence for clinical decision-making. The term "pragmatic", however, is not used in a consistent manner and its definition and measurement need further clarification. Pragmatic trials should be designed to inform clinical practice and policy decisions,

The CRASH trial29, for example, focused on functional outcomes to compare a 2-page case-report with an electronic system for the monitoring of hospitalized patients with chronic heart failure. Methods

Consequently, pragmatic trials may have lower internal validity than explanatory trials and might be more susceptible to bias in their design, conduct, and analysis. Despite these limitations, pragmatic trials can be a valuable source of information for decision-making in healthcare.
The PRECIS-2 tool scores an RCT on 9 domains, ranging between 1 and 5 (very pragmatic). In this study,

In the instance of the pragmatic trials included in this meta-analysis, this was a significant problem since the secondary outcomes were not adjusted to account for differences in baseline covariates.
In addition, pragmatic trials can also have challenges with respect to the gathering and interpretation of safety data. This is due to the fact that adverse events tend to be self-reported, and therefore are prone to delays, errors or coding errors. It is therefore important to improve the quality of outcome assessment in these trials, and ideally by using national registries instead of relying on participants to report adverse events on the trial's own database.
Results
Although the definition of pragmatism may not mean that trials must be 100 percent pragmatic, there are benefits of including pragmatic elements in clinical trials. These include:

Several studies have attempted to classify pragmatic trials using different definitions and scoring methods. Schwartz and Lellouch1 developed a framework to discern between explanation-based studies that confirm a physiological hypothesis or clinical hypothesis, and pragmatic studies that inform the selection of appropriate treatments in the real-world clinical practice. The framework was comprised of nine domains scored on a 1-5 scale, with 1 being more lucid while 5 was more practical. The domains included recruitment of intervention, setting up, delivery of intervention, flexible adhering to the program and primary analysis.
The original PRECIS tool3 was built on the same scale and domains. Koppenaal and colleagues10 created an adaptation of this assessment, called the Pragmascope, that was easier to use for systematic reviews. They found that pragmatic reviews scored higher on average in all domains, but scored lower in the primary analysis domain.
This distinction in the primary analysis domain can be explained by the way most pragmatic trials analyse data. Certain explanatory trials however don't. The overall score was lower for pragmatic systematic reviews when the domains on the organization, flexibility of delivery and follow-up were merged.
It is important to remember that a pragmatic trial doesn't necessarily mean a low quality trial, and indeed there is an increasing rate of clinical trials (as defined by MEDLINE search, but this is neither sensitive nor specific) that use the term 'pragmatic' in their abstracts or titles. The use of these terms in abstracts and titles may suggest a greater awareness of the importance of pragmatism, but it isn't clear if this is reflected in the contents of the articles.
Conclusions
In recent times, pragmatic trials are increasing in popularity in research because the value of real-world evidence is becoming increasingly acknowledged. They are clinical trials randomized that compare real-world care alternatives rather than experimental treatments under development, they involve patient populations that are more similar to the patients who receive routine medical care, they utilize comparisons that are commonplace in practice (e.g. existing medications), and they rely on participant self-report of outcomes. This method can help overcome the limitations of observational research, for example, the biases that come with the reliance on volunteers and the lack of coding variations in national registries.
Pragmatic trials offer other advantages, like the ability to draw on existing data sources, and a greater probability of detecting meaningful differences from traditional trials. However, they may be prone to limitations that undermine their effectiveness and generalizability. Participation rates in some trials could be lower than anticipated due to the health-promoting effect, financial incentives, or competition from other research studies. The necessity to recruit people in a timely fashion also limits the sample size and the impact of many practical trials. Some pragmatic trials also lack controls to ensure that any observed differences aren't due to biases that occur during the trial.
The authors of the Pragmatic Free Trial Meta identified RCTs published up to 2022 that self-described as pragmatic. They assessed pragmatism using the PRECIS-2 tool, which includes the domains eligibility criteria, recruitment, flexibility in adherence to intervention, and follow-up. They found that 14 of these trials scored pragmatic or highly pragmatic (i.e., scoring 5 or higher) in any one or more of these domains and that the majority of them were single-center.
Trials that have high pragmatism scores tend to have more lenient criteria for eligibility than traditional RCTs. They also contain patients from a variety of hospitals. These characteristics, according to the authors,
