Pragmatic Free Trial Meta
Pragmatic Free Trail Meta is an open data platform that allows research into pragmatic trials. It gathers and distributes clean trial data, ratings and evaluations using PRECIS-2. This permits a variety of meta-epidemiological analyses to compare treatment effect estimates across trials of various levels of pragmatism.
Background
Pragmatic trials are increasingly recognized as providing real-world evidence for clinical decision making. However, the use of the term "pragmatic" is not consistent and its definition and evaluation requires clarification. Pragmatic trials are designed to guide clinical practices and policy choices, rather than verify a physiological hypothesis or clinical hypothesis. In the end the aim of pragmatic trials is to make their findings as applicable to current clinical practices as possible. This can be achieved by ensuring their primary analysis is based on the intention-to treat approach (as described in CONSORT extensions).
Many RCTs which do not meet the requirements for pragmatism but have features that are contrary to pragmatism, have been published in journals of various kinds and incorrectly labeled pragmatic. This can lead to false claims of pragmatism, and the term's use should be made more uniform. The creation of the PRECIS-2 tool, which offers a standard objective assessment of practical features is a great first step.
Methods
In a pragmatic study the aim is to inform policy or clinical decisions by showing how an intervention can be integrated into routine care in real-world situations. Explanatory trials test hypotheses regarding the cause-effect relationship within idealised conditions. In this way, pragmatic trials may have lower internal validity than studies that explain and be more prone to biases in their design analysis, conduct, and design. Despite these limitations, pragmatic trials can contribute valuable information to decision-making in the context of healthcare.
The PRECIS-2 tool evaluates the degree of pragmatism within an RCT by assessing it across 9 domains, ranging from 1 (very explicative) to 5 (very pragmatic). In this study, the recruit-ment, organization, flexibility in delivery and follow-up domains were awarded high scores, but the primary outcome and the method of missing data were not at the pragmatic limit. Additionally, studies that are pragmatic may pose challenges to collection and interpretation of safety data. It is because adverse events are typically self-reported and are susceptible to delays, inaccuracies or coding differences. It is therefore important to improve the quality of outcome for these trials, and ideally by using national registries instead of relying on participants to report adverse events on the trial's own database.
Results
While the definition of pragmatism doesn't require that all clinical trials are 100% pragmatist There are advantages of including pragmatic elements in trials. These include:
Enhancing sensitivity to issues in the real world as well as reducing cost and size of the study, and enabling the trial results to be more quickly transferred into real-world clinical practice (by including patients who are routinely treated). However, pragmatic trials may have disadvantages. The right type of heterogeneity for instance, can help a study extend its findings to different settings or patients. Conclusions
As appreciation for the value of real-world evidence becomes increasingly popular and pragmatic trials have gained popularity in research. They are clinical trials that are randomized which compare real-world treatment options instead of experimental treatments in development, they include populations of patients which are more closely resembling the ones who are treated in routine medical care, they utilize comparators that are used in routine practice (e.g. existing medications) and depend on participants' self-reports of outcomes. This approach has the potential to overcome limitations of observational studies, such as the limitations of relying on volunteers and the lack of availability and the variability of coding in national registries.
Pragmatic trials have other advantages, like the ability to use existing data sources and a greater chance of detecting significant differences from traditional trials. However, pragmatic trials may have some limitations that limit their credibility and generalizability. The participation rates in certain trials could be lower than anticipated due to the health-promoting effect, financial incentives or competition from other research studies. The requirement to recruit participants in a timely fashion also restricts the sample size and the impact of many pragmatic trials. Practical trials aren't always equipped with controls to ensure that any observed differences aren't caused by biases that occur during the trial.
The authors of the Pragmatic Free Trial Meta identified 48 RCTs that self-labeled themselves as pragmatic and were published until 2022. The PRECIS-2 tool was employed to evaluate pragmatism. It includes domains such as eligibility criteria as well as recruitment flexibility, adherence to intervention, and follow-up. They found that 14 of these trials scored highly or pragmatic practical (i.e. scoring 5 or higher) in any one or

Trials with a high pragmatism rating tend to have broader eligibility criteria than traditional RCTs that have specific criteria that are not likely to be used in clinical practice, and they contain patients from a broad variety of hospitals.
